Every BTWHS marching band student is required to turn in 3 paper forms and 1 online form as outlined below:
Required Annual Paperwork for Band 2024-25
Every student in our program is required to have the following forms filled out, signed, notarized (when applicable), and turned in to the band office every year:
- Annual band fee contract (your contract will depend on what fee group your student is in)
- Must be signed by student and an adult parent/guardian
- Must be notarized
- Permission Form
- Must be signed by an adult parent/guardian
- Must be notarized
- Drug Screening Consent Form
- Must be signed by student and an adult parent/guardian
- Must be notarized
- Emergency Medical Information form

How do I know which fee group my student is in?
Check out the form tracking spreadsheet – all the student’s fee groups are listed next to their name:

How can I print my student’s forms?
Go to the form tracking spreadsheet. Use the link next to your student’s name to open a folder with PDFs for each paper form.
Can I pick up copies?
Yes! Sign up & attend the last Volunteer Training Meeting on Tuesday, 8/20 at 6pm. Open CutTime, go to “Events”, scroll down to the Volunteer Training Meeting on 8/20 and touch “Volunteers Needed”, then scroll down and touch the green “sign up” button. Everyone that signs up in CutTime for the meeting Tuesday evening will have copies of their forms waiting on them when they arrive for the 6pm meeting!
My forms are completed, signed, and notarized! Where do I turn them in?
Please fold your forms in half long-ways and put them in the drop box by the band office door.
DO NOT SET YOUR FORMS DOWN IN THE OFFICE – THEY WILL GET LOST!
