2023-24 Annual Band Forms

Slightly less confusing than rocket science!

Each student in the BTWHS Band must turn in the following forms to the band office for the this school year:

  1. Fee contract (paper, 1-sided, must be initialed, signed, and notarized)
  2. Emergency Medical/Permission form (paper, 2-sided, must be filled out, signed, & notarized)
  3. Consent to Drug Screening (online only – instructions on page 2)

Where can I get my forms?

Come to any of the 3 Volunteer Training Meetings on the following dates:

  • Friday, 8/11 6-7pm
  • Saturday, 8/12 8:30-9:30am
  • Tuesday, 8/15 6-7pm

We’ll get your volunteer training out of the way, fill out the volunteer application, and then get you your forms and give you time with a notary that will be on hand! You can pick up, fill out, sign & notarize, then turn in your forms in one place. One stop shopping!

Find the section your student is in and click the link to be taken to a folder in our Google Drive with all the available paperwork.  **Unless a director provides you with other instructions!**

Turn in paper forms to band room drop box:

Do not use an envelope!  Fold your form(s) in half longways and put it in the dropbox in the band room office wall where cash/check payments are made. 

Consent to Student Drug Screening – Online Form

  1. Go to athleticclearance.com & select Florida. There are 2 ways to go from here:
    1. Your band student is in another sport and already registered on this site.
      1. Log in with your existing account and add a new clearance with the same school.
      2. Select “band” as the sport. Complete the clearance!
    2. Your band student is NOT in any other sports at BTWHS. Please follow these steps:
  2. Create a new account.
  3. Login with your new account..
  4. Select “Start Clearance”
  5. YEAR: Select 2023-24
  6. SCHOOL: Select “Washington (Pensacola)”
  7. SPORT: Select “Band” – click “next”
  8. Enter student info (PROTIP: this name must exactly match their ‘signature’ later at step 12).  Click “Save and continue”
  9. Enter parent info (PROTIP: this name must exactly match your ‘signature’ later in the process.)  Click “save and continue”.
  10. Enter student medical info, then click “Save and continue”. We do not require a physical!
  11. Enter contact numbers for drug screening notification, then click “Save and continue”.
  12. Student electronically signs the form (see protip in step 8!), then click “Save and continue”. **This field must exactly match the info entered into step 8!**
  13. Parent electronically signs the form (see protip in step 9!), then click “Save and continue”. **This field must exactly match the info entered into step 9!**
  14. The BTWHS Band does not require any further forms to be uploaded!  “Save and continue”.
  15. If it asks for a donation please ignore it and move along.
  16. There is no need to print the confirmation message – we can access clearances online to verify.
