Please complete this form for your student as soon as possible.

Every student in the BTWHS Marching Band is required to have a current Emergency Medical Information Form on file each year. We ask that a student’s grown-up complete the form whenever possible, but we understand that may not always be an option.

Please provide the most current information you have. If your student needs emergency medical care, this is the information we will have available to share with medical personnel until you arrive.

Your responses will be shared only with our medical chaperone and band staff, and with EMTs/paramedics if necessary.

If your information changes later, you should be able to update your response. If you are unable to make changes, please contact band staff and we will update it for you.

Have I already submitted the form?

I will update our Emergency Medical Form tracking spreadsheet as responses come in and as I have time. We are very short-staffed right now, so please give me a little grace if the spreadsheet isn’t updated immediately!

When the box under Emergency Medical Form is checked and turns green, that means your student’s form has been received.

If you do not see your student’s name on the spreadsheet at all, please email me so I can make sure they are added. This is especially important if you have already submitted the form.

What are all those other forms on the spreadsheet?!

Don’t panic! Those forms will be coming soon. I hope to have them ready tomorrow.

For now, please focus on completing and submitting the Emergency Medical Information Form.
