We are at the finish line! There are two final forms that need to be completed for the 2026–27 band year, along with the forms we have previously communicated about.
⏰ THE DEADLINE FOR ALL FORMS IS THURSDAY, AUGUST 27 AT 6:00 PM
ALL required forms must be completed, notarized, and turned in to Ms. Smith by 6:00 PM on Thursday.
Please do not wait until Friday.
Ms. Smith works part time and is in the band office Tuesday, Wednesday, and Thursday. When she arrives for Friday night’s game, she will be busy with game-day responsibilities and will not have time to process forms.
If a student’s required forms are not turned in by 6:00 PM Thursday, the student will not be able to participate.

1. District-Required Consent to Student Drug Screening Form
This is the same form for all students. You will find a PDF version of this form attached to this message.
Did you already complete this form for another sport/organization on campus? Email me with your student’s name and which organization it was for – I’ll notify the front office!
It must be:
- Completed
- Signed in front of a notary
- Notarized
- Turned in to me by 6:00 PM Thursday, August 27
2. Band Fee Contract
The fee contract is different depending on the section your student is in, so please make sure you use the correct contract.
If you’re unsure which fee contract your student needs:
- Open the Band Forms Tracking Spreadsheet.
- Find your student’s name.
- Look at the Fee Group listed next to their name.
- Click the fee group. This will take you to the appropriate Google Drive folder.
- The correct fee contract for your student’s section will be in that folder.
The fee contract must also be:
- Completed
- Signed in front of a notary
- Notarized
- Turned in to me by 6:00 PM Thursday, August 27

- Students and their grownups should both open CutTIme, select “signatures”, open the handbook, read it, then tap to sign. Type your name & submit!
- Permission to Travel Form
- Can be found in the Google Drive folder with your fee contract! See instructions above.
ALL required forms must be completed and turned in by 6:00 PM Thursday.
If you have already turned in a particular form, you do not need to turn it in again.
✍️ NEED A NOTARY?
Ms. Smith will have paper copies of all forms available at my desk.
She will also have packets, forms, and notaries available at the 3rd and FINAL Volunteer Training Meeting on Tuesday evening at 6:00 PM.
This is a great opportunity to get everything completed and notarized in one place!
