A quick reminder that we have three important paperwork requirements that must be completed before your student can participate in Friday night’s football game.

Before you start filling anything out, please take a moment to check our Band Forms Spreadsheet to see which of the three required forms we already have on file for your student.

If a form is already marked as complete, you do not need to submit it again. If something is still missing, please take care of it as soon as possible.
⏰ Emergency Medical Information Form — DUE TODAY AT NOON
If you haven’t already done so, please complete and submit your student’s Emergency Medical Information Form by 12:00 PM today.

📖 Band Handbook — Sign Off in CutTime
Please also log into CutTime and make sure you have reviewed the Band Handbook and completed the required acknowledgment/sign-off. We need each student and at least one grown-up to sign off that they read and understand the handbook!
📝 Permission Form — Due Thursday at 6:00 PM
I’ll be sending out a permission form shortly. This form must be:
- Completed
- Notarized
- Turned in by the end of practice on Thursday at 6:00 PM
🚨 All Three Are Required
In order for a student to participate in Friday night’s football game, we must have all three of the following completed by the required deadlines:
☑️ Emergency Medical Information Form submitted
☑️ Band Handbook acknowledgment completed in CutTime
☑️ Completed and notarized Permission Form turned in by Thursday at 6:00 PM
Need a Notary?
You have a couple of options!
During school hours: We have at least one notary public available in the BTWHS front office, so you can take care of the permission form there during the school day.
At Wednesday’s Volunteer Training: If you sign up in CutTime and attend Wednesday’s volunteer training meeting, I’ll have your permission form ready and waiting for you with the rest of your student’s paperwork. Notaries will be on hand at the meeting, so you can take care of everything in one stop!
Please don’t wait until Thursday afternoon if you can avoid it. The sooner we have these forms completed, the easier it will be for everyone.
Remember: all three requirements must be completed by their respective deadlines for your student to participate in Friday night’s football game.
