Each student in the BTWHS Band must turn in the following forms to the band office for the 2021-22 school year:
- Fee contract (must be initialed, signed, and notarized)
- Emergency Medical/Permission form (must be filled out, signed, & notarized)
- Consent to Drug Screening (fill out online at athleticclearance.com)
- **OPTIONAL: Over the Counter Medication form/Parent Consent for School Health Services

Fold your form(s) in half longways and put it in the dropbox in the band room office wall where cash/check payments are made. Do not use an envelope!
