You’re good – you’re great, in fact!  I am not able to send Charms emails JUST to people that haven’t turned in forms; I swear I wouldn’t keep pestering you if I could avoid it.  Close this and continue being blessed!

Not green?

If you are missing ANY forms, they must be filled out, notarized (if required), and turned in correctly by 4pm Wednesday, 8/23.  Any student with any forms missing or submitted incorrectly after 4pm Wednesday will not be able to travel with us to the game Friday and they will get a 0.

How do I get these forms?

OPTIONS FOR FEE CONTRACTS & EMERGENCY INFO FORM:

PLEASE NOTE: both of these forms must be notarized before being turned in.  We do have at least one notary in the front office of the school – you can come up during the school day and ask Ms. Mitchell to notarize for you.

CONSENT TO DRUG SCREENING FORM – ONLINE ONLY

Step by step instructions to complete this form are below. 

BEFORE YOU BEGIN, PLEASE REMEMBER TO UPLOAD THE **COMPLETED** FORM TO THE SITE. 

DO NOT UPLOAD THE BLANK DOCUMENT!  I NEED THE ONE WITH YOURS AND YOUR STUDENT’S INFORMATION ON IT.

  1. Go to athleticclearance.com Select Florida – there are 2 ways to go from here:
    1. Your band student is in another sport and already registered on this site.
      1. Log in with your existing account and add a new clearance with the same school.
      2. Select “band” as the sport. Complete the clearance!
    2. Your band student is NOT in any other sports at BTWHS. Please follow these steps:
  2. Create a new account, login with your new account, then select “Start Clearance”
  3. YEAR: Select 2023-24; SCHOOL: Select “Washington (Pensacola)”; SPORT: Select “Band” – click “next”
  4. Enter student info (PROTIP: the name here must exactly match their ‘signature’ later at step 12).  Click “Save and continue”
  5. Enter parent info (PROTIP: the name here must exactly match your ‘signature’ later in the process.)  Click “save and continue”.
  6. Enter student medical info, then click “Save and continue”. We do not require a physical!
  7. Enter contact numbers for drug screening notification, then click “Save and continue”.
  8. Student electronically signs the form (see protip in step 8!), then click “Save and continue”. **This field must exactly match the info entered into step 8!**
  9. Parent electronically signs the form (see protip in step 9!), then click “Save and continue”. **This field must exactly match the info entered into step 9!**
  10. FILES page: Scroll down to “Drug Screening 23-24* – at this point you have 2 options:
    1. All digital option: open the PDF I have attached to this email with Adobe Acrobat Reader, type the info into the fields, then save the file.  Upload the saved file to the “Drop file here or click to upload” field.  “Save and continue”
      1. HI, MRS. SMITH HERE AGAIN!  FORGIVE MY SHOUTING, BUT THIS IS SO SO SO IMPORTANT: PLEASE MAKE SURE YOU UPLOAD THE ***SIGNED*** FORM, NOT THE BLANK ONE! 
    2. Paper, then digital option: download the attached PDF, print it, fill it out, sign it.  Take a picture with your phone.  Upload that picture to the “Drop file here or click to upload” field.  Click “Save and continue”.
      1. HI, MRS. SMITH HERE AGAIN!  FORGIVE MY SHOUTING, BUT THIS IS SO SO SO IMPORTANT: PLEASE MAKE SURE YOU UPLOAD THE ***SIGNED*** FORM, NOT THE BLANK ONE! 
  11. There is no need to print the confirmation message – we can access clearances online to verify

Did you already submit a file to athleticclearance but haven’t been cleared yet?

Please see your email – I had about a dozen folks upload a blank document rather than the one that has been filled in.  This is a VERY easy mistake to make – I’ve done it TWICE now myself!  However, I can not clear your student with a blank form.  Please see the directions in your email or the directions above to get this document submitted correctly ASAP.  Not submitting a completed form can prevent your student from going to PHS with us Friday.

Hard deadline

I’ve set two deadlines for forms that have come and gone to no avail.  Wednesday’s deadline is a hard stop.  One of these forms is a permission form; no permission form, no travel.  The online form is required by the district for any stuent in extra-curricular activities.  If you do not submit this form they will remove your student from extra-curricular activites, including band class.
